I am writing to request a copy of my personnel file, as required by California Labor Code § 1198.5.  Please provide me with a complete and accurate copy of all documents in my personnel file,  including but not limited to:

  • All performance reviews 
  • Any disciplinary actions taken against me 
  • Any complaints or grievances made about me 
  • Any investigations or audits involving me 
  • Any payroll records 
  • Any benefits information
  • Any employment agreements or contracts

Please also include any other documents or information that you have maintained in my personnel  file. 

California Labor Code § 1198.5 requires that you provide me with a copy of my personnel file within  30 days of receiving this request. If you have any questions about this request or need additional  information, please contact me at the address or phone number listed above.
