  • All performance reviews 
  • Any disciplinary actions taken against me 
  • Any complaints or grievances made about me 
  • Any investigations or audits involving me 
  • Any payroll records 
  • Any benefits information
  • Any employment agreements or contracts

Please also include any other documents or information that you have maintained in my personnel  file.
